Free client intake form for coaches and consultants in Amsterdam

A coach in Amsterdam needs a client intake form that asks the right questions, shows prices in EUR, uses +31 phone numbers and fits GDPR (AVG). CalendarForm gives you that form free for 250 submissions a month, joined to a booking page that shows times in Europe/Amsterdam (UTC+1, UTC+2 during daylight saving).

What to put on a client intake form in Amsterdam

  1. Full name
  2. Email
  3. Company or role
  4. What do you want to change in the next 3 months?
  5. What have you tried already?
  6. Budget range
  7. Phone: mobile like +31 6 12345678 or a Amsterdam landline like +31 20 xxx xxxx
  8. Preferred language (Dutch and English)
  9. Consent: "I agree to the coach using these details for my booking, as set out in the privacy notice (GDPR (AVG))"

Show the price as text on the booking page, for example "€ 30 for a 60-minute session, pay by iDEAL and card". CalendarForm does not take payment itself, so customers pay you the way they already do in the Netherlands.

What coaches and consultants usually let customers book

CalendarForm lets a coach or consultant share one link for discovery calls and paid sessions, with a short questionnaire so every call starts with context. It is free for 100 bookings a month, with no per-seat fee.

Keep the discovery call short and the form shorter.

Run group workshops as classes with a capacity.

Add buffer time to write notes after each call.

Privacy: what GDPR (AVG) means for this form

Nationally, GDPR (AVG) is GDPR, called AVG in Dutch: collect only what you need and say why.

CalendarForm keeps each business's answers separate, lets you export them to CSV, and does not use them for anything else. Link your own privacy notice in the form's description.

Booking times in Amsterdam

Amsterdam changes its clocks for daylight saving, so a Discovery call at 18:00 stays at 18:00 local time all year. CalendarForm skips times that do not exist on the night the clocks go forward.

Customers booking from abroad still see the time in Europe/Amsterdam, with the zone printed next to it, and their confirmation email repeats it.

Set it up in four steps

  1. Sign up free, pick the closest business type, and rename the starter form to "Client intake form".
  2. Set the time zone to Europe/Amsterdam and add your class timetable.
  3. Edit the questions above, add the GDPR (AVG) consent line, and attach the form to your booking page.
  4. Share your link. Each booking sends an email confirmation with a cancel link, and a reminder 24 hours before.

Questions

Does it connect to Google Calendar?
Calendar sync is on the roadmap. Today, add time off for busy periods so they are not bookable.
Can I add a video call link?
Put your standing meeting link in the service description, which clients see when they book.
Is a free client intake form enough for coaches and consultants in Amsterdam?
For most small coaches and consultants, yes. You get 250 form submissions and 100 bookings free every month; after that, $5 buys 2,000 more submissions or 500 bookings, and packs never expire. Prices on your page can be in EUR.
Does the form follow GDPR (AVG)?
The form collects only the questions you add and shows your consent line. Following GDPR (AVG) is still your job: ask only what you need, say why, and link your privacy notice.
Can I write the form in Dutch?
Yes. Questions, options and the thank-you message are free text, so you can write them in Dutch and English.
