Online intake forms that come with the booking
An intake form collects what you need to know about a new client before you see them: contact details, history, goals, consent. In CalendarForm the intake form is attached to the service, so clients fill it in while they book. It is free for 250 form submissions and 100 bookings every month, then $5 per pack (2,000 submissions or 500 bookings) with no subscription.
How it works in CalendarForm

Yes. Every form also has its own link, so you can send it on its own.
Can I use it as a waiver?
Yes. Add a required checkbox with your waiver wording. CalendarForm stores the answer with the date it was sent.
Does an intake form use up form submissions?
Not when it is filled as part of a booking. That counts as one booking only.
