Free membership registration form for gyms in Spain

A gym in Spain needs a membership registration form that asks the right questions, shows prices in EUR, uses +34 phone numbers and fits GDPR (RGPD) and the LOPDGDD. CalendarForm gives you that form free for 250 submissions a month, joined to a booking page that shows times in Europe/Madrid (UTC+1, UTC+2 during daylight saving).

What to put on a membership registration form in Spain

  1. Full name
  2. Email
  3. Emergency contact name and phone
  4. Medical conditions or injuries
  5. Main goal (strength / weight loss / fitness)
  6. Waiver and gym rules accepted
  7. Phone, in the local format (for example +34 612 34 56 78)
  8. Date of birth, written DD/MM/YYYY in Spain
  9. Consent: "I agree to the gym using these details for my booking, as set out in the privacy notice (GDPR (RGPD) and the LOPDGDD)"

Show the price as text on the booking page, for example "30 € for a monthly membership, pay by Bizum and card". CalendarForm does not take payment itself, so customers pay you the way they already do in Spain.

What gyms usually let customers book

CalendarForm lets a gym publish its class timetable with a capacity per class, take bookings for PT sessions and inductions, and collect a PAR-Q health form at the same time. There is no monthly plan: the first 100 bookings a month are free.

Match the class capacity to the number of bikes or stations.

Put the PAR-Q on the induction booking so it is done before day one.

Use the buffer setting to leave cleaning time between PT sessions.

Privacy: what GDPR (RGPD) and the LOPDGDD means for this form

Nationally, GDPR (RGPD) and the LOPDGDD is GDPR plus Spain's data protection law: show who is collecting the data and why, right on the form.

This membership registration form asks about health, which most privacy laws treat as sensitive. Ask only what the session really needs and say why next to the question.

CalendarForm keeps each business's answers separate, lets you export them to CSV, and does not use them for anything else. Link your own privacy notice in the form's description.

Booking times in Spain

Spain changes its clocks for daylight saving, so a HIIT at 18:00 stays at 18:00 local time all year. CalendarForm skips times that do not exist on the night the clocks go forward.

Customers booking from abroad still see the time in Europe/Madrid, with the zone printed next to it, and their confirmation email repeats it.

Set it up in four steps

  1. Sign up free, pick the closest business type, and rename the starter form to "Membership registration form".
  2. Set the time zone to Europe/Madrid and add your class timetable.
  3. Edit the questions above, add the GDPR (RGPD) and the LOPDGDD consent line, and attach the form to your booking page.
  4. Share your link. Each booking sends an email confirmation with a cancel link, and a reminder 24 hours before.

Questions

Can members book more than one class at a time?
Each booking is one class time. Members can book as many times as they like from your page.
What happens when we pass 100 bookings in a month?
You buy a $5 pack of 500 bookings. Packs never expire, so unused bookings carry over.
Is a free membership registration form enough for gyms in Spain?
For most small gyms, yes. You get 250 form submissions and 100 bookings free every month; after that, $5 buys 2,000 more submissions or 500 bookings, and packs never expire. Prices on your page can be in EUR.
Does the form follow GDPR (RGPD) and the LOPDGDD?
The form collects only the questions you add and shows your consent line. Following GDPR (RGPD) and the LOPDGDD is still your job: ask only what you need, say why, and link your privacy notice.
Can I write the form in Spanish?
Yes. Questions, options and the thank-you message are free text, so you can write them in Spanish.
