What to put on a membership registration form in Hong Kong

  1. Full name
  2. Email
  3. Emergency contact name and phone
  4. Medical conditions or injuries
  5. Main goal (strength / weight loss / fitness)
  6. Waiver and gym rules accepted
  7. Phone, in the local format (for example +852 9123 4567)

  9. Preferred language (Cantonese and English)
  10. Consent: "I agree to the gym using these details for my booking, as set out in the privacy notice (the Personal Data (Privacy) Ordinance (PDPO))"

Show the price as text on the booking page, for example "HK$300 for a monthly membership, pay by FPS and Octopus". CalendarForm does not take payment itself, so customers pay you the way they already do in Hong Kong.

What gyms usually let customers book

CalendarForm lets a gym publish its class timetable with a capacity per class, take bookings for PT sessions and inductions, and collect a PAR-Q health form at the same time. There is no monthly plan: the first 100 bookings a month are free.

Match the class capacity to the number of bikes or stations.

Put the PAR-Q on the induction booking so it is done before day one.

Use the buffer setting to leave cleaning time between PT sessions.

Privacy: what the Personal Data (Privacy) Ordinance (PDPO) means for this form

Nationally, the Personal Data (Privacy) Ordinance (PDPO) is Hong Kong's privacy law: give a Personal Information Collection Statement saying why you collect the data.

This membership registration form asks about health, which most privacy laws treat as sensitive. Ask only what the session really needs and say why next to the question.

CalendarForm keeps each business's answers separate, lets you export them to CSV, and does not use them for anything else. Link your own privacy notice in the form's description.

Booking times in Hong Kong

Hong Kong has no daylight saving, so your timetable never shifts against UTC: UTC+8 every day of the year.

Customers booking from abroad still see the time in Asia/Hong Kong, with the zone printed next to it, and their confirmation email repeats it.

Set it up in four steps

  1. Sign up free, pick the closest business type, and rename the starter form to "Membership registration form".
  2. Set the time zone to Asia/Hong_Kong and add your class timetable.
  3. Edit the questions above, add the the Personal Data (Privacy) Ordinance (PDPO) consent line, and attach the form to your booking page.
  4. Share your link. Each booking sends an email confirmation with a cancel link, and a reminder 24 hours before.

Questions

Can members book more than one class at a time?
Each booking is one class time. Members can book as many times as they like from your page.
What happens when we pass 100 bookings in a month?
You buy a $5 pack of 500 bookings. Packs never expire, so unused bookings carry over.
Is a free membership registration form enough for gyms in Hong Kong?
For most small gyms, yes. You get 250 form submissions and 100 bookings free every month; after that, $5 buys 2,000 more submissions or 500 bookings, and packs never expire. Prices on your page can be in HKD.
Does the form follow the Personal Data (Privacy) Ordinance (PDPO)?
The form collects only the questions you add and shows your consent line. Following the Personal Data (Privacy) Ordinance (PDPO) is still your job: ask only what you need, say why, and link your privacy notice.
Can I write the form in Cantonese?
Yes. Questions, options and the thank-you message are free text, so you can write them in Cantonese and English.
