Yoga and fitness waiver and health form: what to include

Studios ask new students about health and have them accept a waiver before their first class. Collecting both online with the booking saves paper at the front desk.

Health questions most studios ask

  • Experience level
  • Injuries, surgeries or chronic conditions
  • Pregnancy, with an option not to say
  • Anything the teacher should know before class
  • Emergency contact

The waiver part

A waiver is a short statement the student accepts, typically that they take part at their own risk, will tell the teacher about health changes and will follow instructions. Show the full text above a required tick box such as "I have read and accept the waiver".

Waiver wording and how enforceable it is depend on your country. Ask a local lawyer to review your text; this guide is not legal advice.

Collect it with the first booking

  1. Create a form with the health questions and the waiver tick box (or let AI draft it).
  2. Attach it to your trial class or intro offer, and to classes if you want it every time.
  3. Answers arrive with each booking and stay in your dashboard and CSV export.

FAQ

Questions

Do I need a new waiver for every class?

Most studios collect it once, on the first booking or trial class. Attach the form only to the intro service if you do not want regulars to fill it again.

Is an online tick box valid as a signature?

In many places an online acceptance is treated as agreement, but rules differ by country. Check with a local lawyer.
